 APOLOGY MADE AFTER RELEASE OF PATIENT  
 Health officials have apologised to the
 family of a psychiatric patient who    
 killed his wife and committed suicide a
 day after being released from hospital.
                                        
 A inquiry found 50-year-old Michael    
 Hornjr's illness had not been taken    
 seriously enough.                      
                                        
 It was decided that rules were broken  
 when hj was released without warning.  
                                        
 The deaths happened 24 hours after Mr  
 Hornjr was discharged from Queen's Park
 Hospital in Blackburn.
